Your insurance card will be copied for billing purposes.<\/p>\n\n\n\n<p>Payment of insurance co-pays, deductibles and non-covered services is due at the time of service. Payment can be made by check or credit card. We do not accept cash payment. Primary and secondary insurance provided at time of service will be billed as a courtesy on behalf of the patient.
